Please download and print the online forms so they may be completed in the convenience of your own home prior to your first appointment. If you do not have AdobeReader® installed on your computer, Click Here to download.

What you need for your first appointment:

  1. Printed and completed forms.
  2. Comfortable clothing for performing range of motion movements during the exam.
  3. Please refrain from heavy work outs, excessive caffeine, smoking, alcohol, wearing perfumes, colognes & scented lotions prior to your exam.

Payments and Insurance

Investing in your health is one of the best forms of proactive self care! You will be required to pay for your services at the time they are rendered. We accept cash, check, credit and even HSA’s.

Vitality Chiropractic is not a participating provider for any insurance company. However, if your insurance company does cover chiropractic care it is possible you may receive some reimbursement. We will assist you with that process by printing you a “Superbill,” that you are able to send in.
